This letter is my formal resignation from the Church of Jesus Christ of Latter-day Saints, for and in behalf of ***** who is dead. It is to be effective immediately. I hereby withdraw my consent to being treated as a member and I withdraw my consent to being subject to church rules, policies, beliefs and 'discipline', for and in behalf of Richard Phillips Feynman who is dead. As he/she is no longer a member, their name will be permanently and completely removed from the membership rolls of the church.
According to LDS doctrine, ****** is now aware of the truth because they are deceased. They have been baptized into the LDS church and had temple work done by proxy, and with this letter I now give them the chance to resign by proxy. Being aware that the church handbook says that this resignation "cancels the effects of baptism and confirmation, withdraws the priesthood held by a male member and revokes temple blessings
This resignation should be processed immediately, without any 'waiting periods'.
After today, the only contact I want from the church is a single letter of confirmation to let me know that ****** is no longer listed as a member of the church.
Sincerely,
Your Name
Your address
City & State
For an in behalf of *******

Died
